Young Adults team

The Young Adult's team (YAT) is a Multidisciplinary team set up in 2021 to support school leavers with complex needs within CHO7.

We have a full clinical team comprising of Occupational Therapy, Physiotherapy, Psychology, Social Work, Behavior Support, and Speech and language therapy.

The aims of the multidisciplinary team is:
  1. To provide Multidisciplinary team (MDT) supports to young adults with complex needs upon transitioning from school to day service provision.
  2. To support better, planned, and streamlined services for young adults with complex needs upon leaving school.
  3. To capture the goals and recommendations in the individual’s person-centred plan upon closing an episode of care, where relevant.
Criteria for access to the YAT includes:
  • Young person has left school and is aged 18 - 26 years of age.
  • Young person lives or attends a day service in DSKWW.
  • Young person presents with complex disability related needs, which requires support from 2 or more disciplines on the team, and whose needs would not be more appropriately addressed within the framework of a Primary Care or Mental health Service.
  • The day service the young person attends or is due to attend, does not have provision of multi-disciplinary support as part of their service level agreement. Where a day service has discipline specific clinical support available, this should be exhausted prior to a referral to the Young Adults team.
