What is the Integrated Care Programme for Older People (ICPOP) Community Specialist Team?

The Integrated Care Programme for Older People (ICPOP) Community Specialist Team provides multidisciplinary support and care to older adults, in the community, for a defined period of time.

There is some local variation as to what teams call themselves. For this questionnaire all teams will be called the ICPOP team.

The aim of each ICPOP team is to provide care and support that:

• meets each person’s needs, wishes and preferences.
• is provided quickly.
• is well-organised and coordinated.
• feels well-communicated, with all services connected and sharing information (with permission).
• improves the person’s overall care experience, quality of life and wellbeing.

Who can fill out this questionnaire?

• Anyone who has attended the ICPOP team.
• A family member or friend helping someone who has attended the ICPOP team complete the questionnaire.
• A family member or friend who cares for someone who has attended the ICPOP team, sharing their own experience as a carer with the service.

About this questionnaire

The information you tell us is anonymous. This means we cannot identify you from your feedback. Please do not tell us the names of family members or staff. 

We may use some of your stories for presentations, reports, training and other (for example promotional material). You will not be identified in these stories.
