Your Voice Matters questionnaires ask people about their experience of health care services.
We want to find out what is working well, and where we can improve.
- Participation is voluntary.
- We will not ask you for any personal information such as your name, address or date of birth.
- Please do not name any people involved in your care.
- The questionnaire is anonymous. That means we cannot identify you from your responses or provide individual feedback.
- By completing a questionnaire you consent to your experience being shared anonymously.
