CH CDLMS SRF Disability Day Phase II Resources
Community Healthcare, Cavan Donegal Leitrim Monaghan Sligo Disability services were awarded Service Reform Funding (SRF) in 2018 to roll out a regional reform programme (2018 to 2021). The programme subsequently was awarded additional funding for a Phase II of the programme in September 2021. The programme aimed to support the move away from institutional models of care to community based person centered models of service where people with an intellectual disability can live valued and inclusive lives within their local community.
