The Time to Move On from Congregated Settings - A Strategy for Community inclusion report was published and adopted as a national priority in 2012. The report defined congregated settings as 'where 10 or more people with a disability live together in a single unit or in accommodation that is campus based'. The report found that there is a lack of basic privacy and dignity for the residents living in these settings and they often lead isolated lives away from the community, their family and friends.There are around 60 service providers delivering some residential services in congregated settings. This includes services managed by the HSE, voluntary service providers and private providers. The HSE is working in partnership with the disability service providers and other stakeholders to provide support and guidance on the implementation of the policy. These documents have been developed to support services providers to implement the Time to Move on policy.
Literature Review on Models of residential supports for those with significant specialised support needs
This document gives a summary of international strategies, policies, reports and research papers reviewed to support the work of the Work stream 4: Location Review under the Time To Move On Sub-Group. This review aims to gather evidence on the challenges, the outcomes, benefits and recommendations arising in relation to models of residential supports for those with significant specialised support needs.
Review Group Report:Supporting people with disabilities and significant residential support needs
This review was undertaken to examine the specialist residential support needs of people currently living in congregated settings, in order to identify the appropriate approaches to supporting these individuals in line with the Time to Move On policy. The review acknowledging that people with disabilities living in the community will also at times have significant specialist support needs, has sought to reflect and inform the provision of services for all people with significant specialist support needs in the final recommendations.
Drawing on research evidence and consultation the report identifies guiding principles for best practice, a model of care and supporting care pathways for all people with significant specialist support needs. The implementation of the model of care and supporting pathways will support people at the lowest level of complexity to achieve positive outcomes, be person and family centred, enable individuals to enjoy equity of opportunity and participation and ensure that services are responsive, targeted, efficient and cost effective. The report acknowledges the need for strong leadership, joint working and a shared vision with a focus on the individual as well as increasing the service capacity to meet the needs of people with significant specialist support needs. The report sets out three recommended Delivery Strands with identified priority actions and outcomes.
Community Living Transition Planning Toolkit Nov 2018
The Community Living Transition Planning (CLPT) Toolkit aims to guide service providers through the process of supporting each person to develop their plan to move into the community. It will equip service providers with an understanding of what is required to effectively support people to plan their transition to the community.
Project Action Plan Template 2018 version
This document is a template that service providers can use to develop their own specific project action plan. The template guides services to set out their current configuration and their future proposed structure to enable them to plan how to transition and from an institutional model of care to a community based service. The Project Action Plan identifies the eight key work streams that need to be managed in order to drive the change in services and includes a pre-populated work plan for each of these work streams that services can adapt for their specific project and use as a project management tool . The key work streams covered are: housing, person centred planning, transitioning, staffing, finance, governance, communications and training. The template also includes a range of practical project management tools.
Supporting People with Disabilities To Access Appropriate Housing In The Community: A Guidance Document
This document provides information for disability service providers on the mechanisms and funding options available to secure homes for people with disabilities. The Document provides an overview of the various approaches which service providers can consider in order to determine the most appropriate option for the person with a disability they are supporting.
Making A Home: A practical guide to creating a home and moving to the community
This document has been developed as a resource for service providers and other stakeholders who are supporting people with disabilities to move from congregated settings to their new homes in the community. The document maps the practical steps that may need to be navigated from the time a home is selected through to when the person moves in, essentially the process of “making a home”. The document signposts relevant additional information, easy reads and resources and includes a number of practical checklists that can be used to support the process.“ An easy read version of the document is also available.

Communication Plan and Key Messages
This document supports the development of a communication and engagement strategy and relevant key messages. The key messages can be tailored for each specific group of stakeholders, to ensure that the focus of the message, language and medium used are appropriate and effective.
Communication Plan Stakeholder Mapping Tool
This mapping tool identifies many of the stakeholders that may need to be considered and included in the Communication and Engagement Strategy. The tool prompts the providers to consider Who, What, When, How in terms of Who is the stakeholder, What is the message relevant to them When should engagement and communication happen, How should this be done in terms of the appropriate method of communication and by whom.
