Cuan Aoibheann is a chronic disease hub based in Phoenix Park.
The objective of the hub is to provide ready access to diagnostics, HSCPs and specialist support to GPs in the community in managing patients with chronic disease. A GP referral is required for most services.
Cuan Aoibheann provides specialist team services for patients with heart disease, COPD, asthma and diabetes type 2, who live in Finglas, Blanchardstown or Blakestown.
The specialist teams include; specialist nurses, physiotherapists, diabetes dietetic service, diabetes podiatry service and the Stop Smoking Advice service. They may also include consultant provided and led specialist clinical services. The hub also provides self-management support services. Such services include pulmonary rehabilitation, cardiac rehabilitation, diabetes structured patient education, diabetes prevention and weight management programmes.
