The social worker promotes change, problem solving in human relationships, encourages help along with promoting advocacy and empowerment.
Working with clients on an individual and Group Basis on issues such as adjustment to changing life situations as a result of illness, bereavement, Separations, Trauma, Suicide.
Support will be offered to families, older persons, vulnerable adults, persons with disabilities. Referrals to appropriate long term services will be offered where necessary.
Exclusions - Who we don't see
- Clients presenting with child protection and welfare concerns. These families should be referred to Children and Family Services
- Clients presenting with Elder Abuse issues and significant mental health issues- These clients will be referred to the existing services where possible and appropriate. Adult Safeguarding & Protection Team 012164511 or safeguarding.cho@hse.ie
- Clients already linked to an existing Social Work service- voluntary or statutory
