Nursing
The Public Health Nursing Service provides a wide range of personal health and social care to older people, people with acute and chronic illnesses and those with disabilities who are medical care holders.
Clinical nurse services include wound dressings, assessment of care needs, care of the terminally ill and liaising with acute services to facilitate early hospital discharges and continuity of care.
Public health nurses also provide advice and support to all parents and families irrespective of eligibility, including information on infant feeding, child development, immunisations and vision and hearing screening.

Occupational Therapy
Occupational Therapists help people overcome limitations caused by injury, illness, disability or the effects of aging. The Occupational Primary Care service aims to enable clients to live as independently as possible, for as long as possible in their own homes.
They provide assistive devices and advice on adaptations to ensure a safe and accessible home environment
Where assistive devices are prescribed by the OT, these are provided free of charge to medical card holders and long term illness book holders.

Social Work
Social Work services are delivered within each network. Social Work provide early intervention in the areas of children & family, assist in hospital discharge planning, care planning with older people or vulnerable adults living alone with no or low level of social supports and risk exists concerning their safety, carers support. Referrals to Primary Care Social Work Services can be through all members of the Primary Care Team/ GP or self-referral. All members of the public can access this service.

Psychology
The primary care Psychologist provides services for children and adults in order to address a broad range of psychological and mental health difficulties. Typical areas that the psychologist can help with include parenting advice/support for child-related difficulties, family problems, therapeutic interventions for depression, anxiety, trauma, adjustment and loss, stressful life events, relationship and other mental health issues. The goal of the Psychology service is to provide psychological assessments and interventions in a confidential and supportive environment in order to alleviate psychological distress and to improve psychological functioning. Referrals may be made to psychology through other members of the Primary Care Team or by your GP.

Home Help
Home Support is a partnership between informal (family, friends, neighbours) voluntary and statutory services in meeting a person's goal to remain at home. The service provides assistance with personal activities of daily living and priority is given to all aspects of personal care such as getting in and out of bed, washing and showering, continence care and toileting, dressing and un-dressing, medication prompt. However your local Health Office is not legally obliged to provide these services.
A Home Support assessment is carried out to identify any personal self care deficits which impact on the persons ability to remain at home. The assessment is carried out by the Public Health Nurse or the Community Nurse. It is done using a standard clinical assessment process as set out nationally.
The allocation of home support hours is based on a standard clinical assessment process based on the level of dependency of each individual together with family and home circumstances. this ensures equity of service to all applicants within the level of resources available. If the demand for Home Support is greater than the resources available you may be placed on a waiting list. This list is prioritised according to clinical risk assessment. Review assessments are routinely carried out to ensure that care needs are being met and resources used as efficiently and effectively as possible.
