Extra support for your health - Kilkenny & Carlow

Our records show that you might benefit from more support to manage your health.

We’re offering you free, extra support through services close to your home. These services can help you learn ways to manage your health and hopefully avoid trips to the hospital.

Examples of these services are:

  • The SMILE programme: this programme, run by Caredoc in partnership with the HSE and Sláintecare, offers virtual health management for people with chronic diseases. SMILE (Supporting Multi-morbidity Self-care through Integration, Learning, and eHealth) combines nursing expertise with remote health monitoring, helping you to manage your conditions from home and live more independently.

  • ALONE: this national organisation provides support to older people, including support and befriending services, to age well at home.

  • Community Specialist Teams: these HSE teams provide specialist support for older people and those with chronic disease (hse.ie/ecc).

This will help you learn new ways of managing your health and may reduce the need for trips to the local Emergency Department.

What to Expect

HSELive will call you to talk about the free, local health and social care support options available. They'll explain everything and give you the choice to join.

During the call, we will:

  • Confirm who we are and explain our role
  • Let you know why we’re calling
  • Explain the community supports available to help you better manage your health
  • Give you the choice to take part or not
  • Outline the next steps if you decide to join

We use hospital records to identify patients who may benefit from extra health support. All information is confidential and is only accessed by a healthcare professional.

This data helps us reach out to you, invite you to take part and develop your care plan.

Participation is voluntary. You can opt out at any time by informing HSELive on 1800 700 700. If you choose to leave, you will no longer receive these extra health supports.

All existing services you currently receive will continue as usual.
