This service is suitable for children who do not have complex needs.
We are not a diagnostic service.
Referring your child to this service
Children must:
- be under 18 years
- have functional difficulties that stop them taking part in their daily activities - these can be fine or gross motor skills difficulties or problems with their self-care or daily living skills
- live in our catchment area
- not be attending another service or be on a disability service waiting list
We accept referrals from parents, teachers, or health professionals.
Submitting the referral form
Complete the referral form.
Describe the difficulties your child experiences.
Post the referral form to our address. We do not accept completed referral forms by email.
Referrals will only be processed when we receive the fully completed referral form.
After we receive a referral
An Occupational Therapist (OT) will review the referral.
If your referral is accepted you will be given a date for a screening appointment.
You will be informed in writing if your referral has not been accepted. We will send your referral to the correct service.
What to expect from our service
We use a Family Centered progressive Layered Care or Tiered approach to service delivery.
Pathways
When we receive your referral, you will be listed for a screening appointment.
If your child is aged under 23 months
When we receive your referral your appointment will be scheduled within 3 months.
If your child is aged between 2 and 18 years
When the referral is accepted you will be listed for a screening appointment.
Screening clinics are held at the beginning of every month.
Following this you and your child may be directed to a:
- monthly parent and child clinic or workshop
- more suitable service, for example a Children's Network Disability Team
- waiting list for individual treatment
Parent and child clinics and workshops
These are held once a month and include:
- Gross Motor clinic
- Fine Motor clinic
- Sensory Motor clinic
Other regular workshops include:
- Activities of daily living/self care skills workshop
- Handwriting workshops
- Parent education workshops
