This page contains information to help you apply for vaccination records for the Swine Flu pandemic vaccination campaign only.
The National Immunisation Office cannot help you to access or amend COVID-19 vaccine records.
The Swine Flu vaccination campaign took place in Ireland in 2009 and 2010. Swine Flu vaccines were offered in Mass Vaccination Clinics, through GPs and in school settings. The HSE National Immunisation Office do not have access to records for Swine Flu vaccines administered through GP services.
How do I apply?
You can request a copy of Swine Flu pandemic records by post or email.
Please send:
- Letters to Records Request, HSE National Immunisation Office, Unit 8/9 Manor St Business Park, Manor St, Dublin 7
- Emails to immunisation@hse.ie please ensure you have "Swine Flu pandemic record request" in the subject line of your email
Please note we cannot accept responsibility for the security of transmission of information by email from your account.
What do I need to include in my request?
We will need the following information to process your request
- Name
- Date of Birth
- Address
- Place of vaccination (if available)
- Official photographic identification examples include a driving licence or passport
Please note we only need a copy of your ID. Please do not send original documents with your request.
Once your submission has been processed the copy of your ID supplied will be securely disposed of.
Is there anything else I need to be aware of?
Please remember anyone over the age of 18 must apply for a copy of their records themselves.
Anyone under 18 a parent or guardian need to apply for a copy of vaccination records.
Where can I get the rest of my records?
The National Immunisation Office do not have access to any other records. For information about applying for other vaccination records visit our dedicated web page.
