Self-management support is the systematic provision of education and other interventions to increase patients’ skills and confidence in managing their health problems. It improves quality of life and clinical outcomes for patients, while also helping to reduce healthcare utilisation, including hospital admissions.
The curriculum aims to equip our future healthcare professionals with the knowledge and skills to support people in the management of the medical, emotional and social aspects of living with a long-term condition. Facilitating and training our future healthcare professionals to support self-management, is part of the whole system approach set out in ‘Living Well with a Long-term Condition’, the national Framework for Self-management support HSE 2017
