Living Well programme
Living Well is a free 6-week group self-management programme for adults with long-term health conditions.
It helps people to:
- gain confidence
- learn skills to live well with their condition
Living Well is the brand name for the HSE 'Chronic Disease Self-Management programme (CDSMP)' in Ireland. This is based on the Stanford model.
Workshops are delivered by 2 trained leaders. One or both leaders will have firsthand experience living with a long-term health condition.
Living Well helps people develop the skills and confidence to manage their health condition better. It does not give clinical advice or information.

Chronic Disease Management (CDM) in General Practice
This online course is for all staff involved in delivering and supporting CDM through general practice.

Healthcare professionals play an important role in making sure medicines are safe and taken correctly. The aim of this campaign is to encourage those taking medication and their caregivers to take an active role in managing their medication.

Exercise videos
HSE physiotherapy exercise videos are suitable for people living with a long-term or chronic conditions.
The videos:
- are for people who can exercise safely at home
- include a variety of 10-minute warm up, chair and gentle exercise videos
- longer full aerobic and resistance exercise videos

Making Every Contact Count programme
Making Every Contact Count (MECC) is a behaviour change programme. During routine consultations, health professionals ask patients or service users about lifestyle behaviours. related to chronic conditions.
The aim is to address factors that can lead to chronic conditions. It is also to support patients in making lasting positive changes in their behaviour.
MECC gives healthcare professionals the knowledge and skills to do a brief intervention with patients.
The training course consists of 8 x 30-minute online modules. There are 8 modules in the course. Each lasts 30 minutes and is available on HSeLanD.
These modules are:
- introduction to behaviour change
- tobacco free
- alcohol and drug use
- get Ireland active
- healthy food for life
- talking about overweight and obesity
- promoting mental health and wellbeing
- skills into practice
When you have completed the course you can do the 'Enhancing your Brief Intervention Skills workshop' in person or online.
Framework for self-management support
The 'Living Well with a Chronic Condition: Framework for Self-management Support' is a framework and implementation plan for self-management support for chronic conditions. It shows how the HSE intends to implement self-management support for 4 conditions.
These are:
- COPD
- asthma
- diabetes
- cardiovascular disease
The framework includes:
- information on why self-management support is important
- recommendations for different health conditions
- details on the implementation plan
