Role of Clinical Nurse Coordinator for Children with Life Limiting Conditions

Clinical Nurse Coordinators (CNCs) provide continuity of care for children with life limiting conditions, particularly children with palliative care needs, and their families.

Patient caseloads are coordinated in collaboration with local healthcare professionals/carers in acute and community care settings.

CNCs cover set geographical areas. A child should be referred to the CNC who covers the area in which the child lives, regardless of what hospital they attend.

Referral process

• Any health care professional may make a referral to a CNC.
• Complete the referral form with as much information about the child as possible. It should include a comprehensive list of the professionals involved in the child’s care, both hospital and community. Email the CNC before completing the form if you are unsure of the process, or the criteria for referral.
• The child’s primary consultant and parents/guardians must be aware and agreeable to the referral.
• Each referral is reviewed by the CNC as soon as possible.
• The CNC may need further information. They will report back to the referrer advising if the referral meets the acceptance criteria.
• If the referral is accepted, a triage tool is used to determine the urgency of the referral and how soon contact should be made with the child and family.
• If the referral is not accepted, a reason will be given and the primary consultant will be informed. The child may be referred again if their condition or prognosis change.
